- Restorative codes: 2311x (permanent anterior resin), 2331x (permanent bicuspid resin), 2332x (permanent molar resin); where x is the number of surfaces
- Emergency Exam 01205, Specific Exam 01204
- Extraction code: 71101 (simple) 71201 (complex)
- Root canal treatment on adult tooth: 331×1; where x is the number of canals
- Crown: 27201 (all ceramic crown) 27211 (PFM crown)
- Recement crown: 29101
- Nitrous: 9241x where x is the number of units of time (per 15 mins)
- Maxillary bruxism appliance (nightguard): 14611
- Desensitization 4130x where x is the number of units of time (per 15 mins)
