  1. X-ray:0211x (periapical radiograph), 0214x (bitewing radiograph), 02601 (panoramic); where x is the number of images. For example. the code 02142 is the billing code for 2 bitewings
  2. Restorative codes: 2311x (permanent anterior resin), 2331x (permanent bicuspid resin), 2332x (permanent molar resin); where x is the number of surfaces
  3. Emergency Exam 01205, Specific Exam 01204
  4. Extraction code: 71101 (simple) 71201 (complex)
  5. Root canal treatment on adult tooth: 331×1; where x is the number of canals
  6. Crown: 27201 (all ceramic crown) 27211 (PFM crown)
  7. Recement crown: 29101
  8. Nitrous: 9241x where x is the number of units of time (per 15 mins)
